Are your services HSA/FSA Eligible?

Yes! Our services can be paid for with your HSA/FSA card, we can also provide a superbill if requested.
What is your Cancellation Policy?
We understand that fertility treatment schedules can change. To ensure availability for all patients and to respect our nurses’ time, the following cancellation policy applies:
Cancellations More Than 24 Hours Before Appointment:
Patients who cancel their scheduled appointment more than 24 hours before the visit will receive a full refund.
Cancellations Within 24 Hours:
Appointments cancelled less than 24 hours before the scheduled visit will be charged the full visit fee.
Fertility Provider Schedule Changes:
If a cancellation within 24 hours occurs due to a change made by the patient’s fertility provider, the patient will receive a full refund, provided that written confirmation of the change from the fertility provider is submitted.
Does the nurse provide any medications or injection supplies?
Patients are responsible for obtaining all medications and injection supplies prior to the scheduled appointment.
Nurses from The Fertility Collective do not provide medications, syringes, needles, alcohol pads, or other injection supplies. These items must be supplied by the patient and prescribed by their fertility provider.
To ensure a smooth and timely visit, patients are expected to have all medications and required supplies available and ready at the time of the appointment.
If required medications or supplies are not available at the time of the visit, the appointment may still be considered completed and the full visit fee may apply.
We currently have nurse availabilty in the greater Phoenix area and surrounding cities. When an appointment request is made we do our best to pair you with a nurse, but in the event that a nurse is not in close proximity to the requested area we will reach out as soon as possible to let you know. We are always expanding our nurse network!

Who will be providing the injection services?
All injection services provided through The Fertility Collective are performed exclusively by licensed Registered Nurses (RNs) with experience in the fertility and reproductive health field. Our nurses are trained in fertility medication protocols and injection techniques commonly used in assisted reproductive treatments. This ensures patients receive care from qualified professionals who are familiar with the unique timing, medications, and procedures involved in fertility treatment.
