Getting Started
Click "Book Now" to view available appointment times and select the option that works best for you. Once your appointment is scheduled, you'll be prompted to create a secure client profile, complete your medical intake forms, and review and sign the necessary consent documents. Completing these forms before your appointment helps ensure we can provide you with the most personalized and efficient care possible. (Note: for time sensitive injections please schedule your visit for 15 minutes prior to injection time). For your safety, we also require a copy of your prescription—this helps our nurses prepare properly and provide the highest level of care during your appointment.
What Happens Next
After your request is submitted, you will be connected with a highly skilled fertility nurse in your area. Your nurse will reach out to you through the Jane app messaging system and ensure every detail aligns perfectly with your needs. Through the Jane app, you can effortlessly connect with your nurse for any last-minute changes, updates, or important details.
